Title: ONGOING MONITORING
Wording H - AS PER WAC 246-272A AND JEFFERSON COUNTY CODE 8.15 ALL ONSITE SEWAGE SYSTEMS REQUIRE THAT A RESTRICTIVE COVENANT REGARDING THE MONITORING OF THE ONSITE SEPTIC SYSTEM BE RECORDED TO THE PROPERTY TITLE. THE PROPERTY OWNER SHALL ASSURE THAT MONITORING IS PROVIDED BY AN APPROVED ENTITY AT THE FREQUENCY DEFINED PER STATE WAC 246-272A AND JEFFERSON COUNTY CODE 8.15 AS ADOPTED OR AMENDED. A COPY OF THE RECORDED OPERATIONS AND MONITORING AGREEMENT IS REQUIRED PRIOR TO FINAL APPROVAL OF THE SEWAGE DISPOSAL PERMIT

Wording TRENCH TO BE EXCAVATED TO 48" DEPTH AND THEN BACKFILLED WITH ASTM 33 TYPE SAND TO 24". 6" OF SAND SHALL BE PLACED UP THE SIDE WALLS ALL AROUND THE BED TO PREVENT SHORT CIRCUITING FROM THE GRAVEL BED TO THE EXCESSIVELY COARSE SOILS.

Wording Approval of this sewage disposal permit does not preclude the permit holder from complying with the Unified Development Code for other/future development on the site.

Title: minimum land RESTRICTIVE COV
Wording H - This permit is granted based on the recorded RESTRICTIVE COVENANT combining lots 11 & 12 of block 7, South Port Townsend. to meet the minimum land area required under WAC 246-272A Onsite Sewage Code. COVENANT recorded 6/28/19 under AFN 625172

Title: risers
Wording Risers are required to grade with secured lids over both compartments and over the outlet of the septic tank for inspection and maintenance/monitoring. Minimum 20" riser over inlet.

Title: System Capacity-no ADU
Wording This system as designed and approved is sized for only one single family residence. It is not sized for an Accessory Dwelling Unit (ADU). The minimum daily design flow per residence is 240 GPD.

Title: Water line setback
Wording 10' separation required between a Water line and all portions of the onsite sewage system; effluent transport line, tanks, treatment and disposal components EXCEPT as specifically identified in the design. Sewer line shall be cased in schedule 40 from the foundation of the structure to a point at least 10' from all septic system components.

Title: VIOLATION-limited permit life
Wording This permit was issued to correct a violation of WAC 246-272A. The permit must be completed within 90 days of the date of issuance.

Title: PUMP CHAMBER
Wording Pump chamber shall be water tight and a minimum capacity of 1000G. It shall be equipped with a control panel outside within line of sight of the pump chamber. The panel shall have an audio and visual alarm, a cycle counter and a riser to grade. It shall be UL listed and meet L&I requirements.

Title: SCREEN REQUIRED
Wording A screen shall be provided either at the outlet of the septic tank or around the pump or at both locations. A pump shroud is required if only an outlet baffle filter is used.

Title: O & M Manual required
Wording Before final approval is given, the designer shall provide an operations and maintenance manual to the property owner and the Health Department. The manual must instruct the owner of the on site sewage system on the ways to properly operate and maintain all components of the system.

Title: System capacity/uses
Wording Approval of this permit does not assure the existing septic system has capacity for all uses allowed by current code.

Title: Designer Notification/inspections
Wording Designer must be contacted prior to start of construction and for inspections during installation. DESIGNER IS REQUIRED TO DO A PRECOVER INSPECTION ON ALL TYPES OF SYSTEMS.

Title: Installation Inspection
Wording Notification of the start of construction shall be faxed or emailed to Jefferson County Public Health ONE WORKING DAY prior to start.

Title: No Building Permit Guaranteed
Wording Approval/issuance of a sewage disposal permit or installation of a septic system does not guarantee the approval of other development or a building permit on this site. Future buildings that require connection to an on-site sewage system (OSS) shall only be approved if the OSS meets the current standards and codes in effect at the time of the building application.

Title: Code Compliance
Wording All construction and development activities must comply with all permit conditions, Washington State and Jefferson County Codes and Recommended Standards and Guidance documents in effect when the permit is issued.

Title: APPROVAL FOR REP/EXP OF SEP ONLY
Wording This approval is for repair/expansion of existing septic system only. Any future permits on this site are subject to review for consistency with applicable codes and ordinances and does not preclude review and conditions which may be placed on future permits.

Wording The site plan as submitted with the septic repair/expansion application on July 18, 2019 has been reviewed for consistency under the UDC by Jefferson County Department of Community Development.
