THE PROPERTY OWNER SHALL ASSURE THAT MONITORING IS PROVIDED BY AN APPROVED ENTITY AT THE FREQUENCY DEFINED PER STATE WAC 246-272A AND JEFFERSON COUNTY CODE 8.15 AS ADOPTED OR AMENDED. A COPY OF THE RECORDED OPERATIONS AND MONITORING AGREEMENT IS REQUIRED PRIOR TO FINAL APPROVAL OF THE SEWAGE DISPOSAL PERMIT

Wording This onsite sewage system is designed for domestic strength wastewater only. Disposal of any other waste strength is considered a violation of this permit.

Title: No Building Permit Guaranteed
Wording Approval/issuance of a sewage disposal permit or installation of a septic system does not guarantee the approval of other development or a building permit on this site. Future buildings that require connection to an on-site sewage system (OSS) shall only be approved if the OSS meets the current standards and codes in effect at the time of the building application.

Title: Code Compliance
Wording All construction and development activities must comply with all permit conditions, Washington State and Jefferson County Codes and Recommended Standards and Guidance documents in effect when the permit is issued.

Title: Contaminated Material - offsite
Wording H - The material that is removed from the system that has come into contact with waste water must be treated/decontaminated prior to final disposal. The material from the system shall be removed and stockpiled to allow for pathogen die-off. The material shall be placed in 6"-8" lifts, each lift covered with lime. The location for the stockpile shall be approved by the Health Officer and shall meet all requirements of an onsite sewage system (setbacks from surface waters and wells and vertical separation, etc.). It shall be covered and protected from erosion (covered with plastic or several 6 inches of uncontaminated soil and covered with straw, etc.). After the material has been stockpiled for a minimum of one year, the material may be used without site restrictions. Any alteration or modification from this process must be reviewed and approved by the Health Officer prior to start of work.

Title: SYSTEM FLOWS-NO HOLD
Wording This residence is limited to the existing 3 bedrooms, the onsite sewage system repair is being sized for additional flows ONLY to accomodate reported and documented water use. The design approval does not comply with code requirements for expansion in a critical aquifer recharge area. Additional treatment will be required to increase the number of bedrooms served by this system.

Title: SYSTEM DIMENSIONS-NO HOLD
Wording This system consists of both a conventional bed, width 6', and a conventional trench, width 3', to achieve the required square footage for the soils identified.

Title: BARRIER TO EXISTING OSS-HOLD
Wording H - As noted in the design - Where the existing septic system is intersected a minimum of 1' of the existing drainfield shall be removed and barrier material, bentonite, shall be placed to assure no short circuiting between the new and existing systems.
