Title: ONGOING MONITORING
Wording H - AS PER WAC 246-272A AND JEFFERSON COUNTY CODE 8.15 ALL ONSITE SEWAGE SYSTEMS REQUIRE THAT A RESTRICTIVE COVENANT REGARDING THE MONITORING OF THE ONSITE SEPTIC SYSTEM BE RECORDED TO THE PROPERTY TITLE. THE PROPERTY OWNER SHALL ASSURE THAT MONITORING IS PROVIDED BY AN APPROVED ENTITY AT THE FREQUENCY DEFINED PER STATE WAC 246-272A AND JEFFERSON COUNTY CODE 8.15 AS ADOPTED OR AMENDED. A COPY OF THE RECORDED OPERATIONS AND MONITORING AGREEMENT IS REQUIRED PRIOR TO FINAL APPROVAL OF THE SEWAGE DISPOSAL PERMIT

Wording WAC 246-272A REQUIRES ANNUAL MONITORING OF THE ONSITE SEWAGE SYSTEM WHEN IT IS SERVING A FOOD SERVICE ESTABLISHMENT.

Title: domestic strength waste
Wording This onsite sewage system is designed for domestic strength wastewater only. Disposal of any other waste strength is considered a violation of this permit. Proposal for additional food service activities including hours of operation, items served shall require review and additional information and may require additional hydraulic capacity, additional treatment such as a grease trap and/or other treatment device to address high strength waste prior to approval.

Title: BOD-WASTE STRENGTH MONITORING
Wording Waste Strength (BOD, TSS, Grease and oils) is to be monitored quarterly for the first year of operation and annually thereafter. Responsibility is that of the owner. Results are to be submitted to the Jefferson County Health Dept.

Title: design flow/water usage monitorin
Wording Exceeding the permitted design flow of 720 gal/day peak flow shall constitute a violation of this permit. Water use/flows shall be submitted to Jefferson County Public Health upon request. Average water use should be in the range of 475-540 gal/day.

Title: risers
Wording Risers are required to grade with secured lids over both compartments and over the outlet of the septic tank for inspection and maintenance/monitoring. Minimum 20" riser over inlet. 24" Access risers shall be provided on the pump chamber and treatment unit for service and monitoring.

Title: ATU maintenance
Wording A maintenance and management contract is required with the manufacturers authorized representative. Maintenance of the system shall be completed by a manufacturers authorized representative at the frequency required by the manufacturer.

Title: ATU RECORDING/COMPLIANCE
Wording H - The signed Notice for Maintenance and Monitoring of an Onsite Sewage System shall be recorded to the property title prior to final approval. Non-compliance with the contract or maintenance requirements is a violation of this permit.

Title: protective barriers
Wording H - Permanent barriers are required along/around septic tanks, pump chamber and treatment unit areas to protect from parking, driving, and other land disturbing activities prior to final. Barriers shall be required to protect the primary and reserve drainfield areas when/if the property is developed.

Title: Water line setback
Wording 10' separation required between a Water line and all portions of the onsite sewage system; effluent transport line, tanks, treatment and disposal components.

Title: Setback - Drains
Wording A 30' setback shall be maintained from the disposal component and reserve area to any downslope footing, curtain or interceptor drain or drainage ditch.

Title: VIOLATION-limited permit life
Wording This permit was issued to correct a violation of WAC 246-272A. The permit must be completed within 90 days of the date of issuance.

Title: PUMP CHAMBER
Wording Pump chamber shall be water tight and a minimum capacity of 1,500 gallons. It shall be equipped with a control panel outside within line of sight of the pump chamber. The panel shall have an audio and visual alarm, a cycle counter and a riser to grade. It shall be UL listed and meet L&I requirements. Designer shall provide documentation/statement if intrinsically safe wiring is not installed on this commercial system.

Title: SCREEN REQUIRED
Wording A screened pump vault shall be provided around the pump.

Title: O & M Manual required
Wording Before final approval is given, the designer shall provide an operations and maintenance manual to the property owner and the Health Department. The manual must instruct the owner of the on site sewage system on the ways to properly operate and maintain all components of the system.

Title: System capacity/uses
Wording Approval of this permit does not assure the existing septic system has capacity for all uses allowed by current code.

Title: Designer Notification/inspections
Wording Designer must be contacted prior to start of construction and for inspections during installation. DESIGNER IS REQUIRED TO DO A PRECOVER INSPECTION ON ALL TYPES OF SYSTEMS.

Title: Installation Inspection
Wording Notification of the start of construction shall be faxed or emailed to Jefferson County Public Health ONE WORKING DAY prior to start.

Title: Certified Installer Required
Wording This system must be constructed by an installer certified by the Jefferson County Public Health Department per Jefferson County Code 8.15.120 (9) and state code WAC246-272A-0250.

Title: No Building Permit Guaranteed
Wording Approval/issuance of a sewage disposal permit or installation of a septic system does not guarantee the approval of other development, permit or a building permit on this site. Future buildings or uses that require connection to an on-site sewage system (OSS) shall only be approved if the OSS meets the current standards and codes in effect at the time of the building application.

Title: Code Compliance
Wording All construction and development activities must comply with all permit conditions, Washington State and Jefferson County Codes and Recommended Standards and Guidance documents in effect when the permit is issued.

Title: Contaminated Material - onsite
Wording H - The material that is removed from the system that has come into contact with waste water must be treated/decontaminated prior to final disposal. The material from the system shall be removed and placed as described in the design to allow for pathogen die-off. The material shall be placed in lifts of no more than 6"-8", each lift covered with lime. It shall be covered and protected from erosion (covered with 6 inches of uncontaminated soil or other approved material covered with straw, etc.).

Title: FUTURE DEVEL.ON DF EASEME-NO HOLD
Wording Any development on the property/lot that contains the drainfield easement shall comply with Nitrogen BMP's if required in the UDC based on less than 1 acre remaining available outside the easement area for 901024020.

Title: Stormwater BMPs
Wording The project shall adhere to the Best Management Practices (BMPs) to control stormwater, erosion and sediment during construction. BMPs shall address permanent measures to stabilize soil exposed during construction, and in the design and operation of stormwater and drainage control systems.
