Title: ONGOING MONITORING
Wording H - AS PER WAC 246-272A AND JEFFERSON COUNTY CODE 8.15 ALL ONSITE SEWAGE SYSTEMS REQUIRE THAT A RESTRICTIVE COVENANT REGARDING THE MONITORING OF THE ONSITE SEPTIC SYSTEM BE RECORDED TO THE PROPERTY TITLE. THE PROPERTY OWNER SHALL ASSURE THAT MONITORING IS PROVIDED BY AN APPROVED ENTITY AT THE FREQUENCY DEFINED PER STATE WAC 246-272A AND JEFFERSON COUNTY CODE 8.15 AS ADOPTED OR AMENDED. A COPY OF THE RECORDED OPERATIONS AND MONITORING AGREEMENT IS REQUIRED PRIOR TO FINAL APPROVAL OF THE SEWAGE DISPOSAL PERMIT

Wording H - This permit is granted based on the recorded RESTRICTIVE COVENANT combining Lots 138 and 139 of Seventh Avenue Acre Tracts to meet the minimum land area required under WAC 246-272A Onsite Sewage Code and tie the lot with the building to the lot with the onsite sewage system together. COVENANT to be recorded prior to final with the Jefferson County Auditor.

Wording This onsite sewage system is designed for domestic strength wastewater only. Disposal of any other waste strength is considered a violation of this permit.

Title: BOD-WASTE STRENGTH MONITORING
Wording Waste Strength (BOD, TSS, Grease and oils) is to be monitored quarterly for the first year of operation and annually thereafter. Responsibility is that of the owner. Results are to be submitted to the Jefferson County Health Dept.

Title: design flow/water usage monitorin
Wording Exceeding the permitted design flow of 360 gal/day occasional peak flow shall constitute a violation of this permit. Water usage monitoring shall be conducted daily for the first two months & weekly thereafter. These monitoring results shall be submitted to the Jefferson County Health Dept. no less than quarterly. Average flows should not exceed 240 gpd.

Title: risers
Wording Risers are required to grade with secured lids over both compartments and over the outlet of the septic tank for inspection and maintenance/monitoring. Minimum 20" riser over inlet.

Wording H - Permanent barriers are required along/around primary and reserve drainfield areas to protect from parking, driving, and other land disturbing activities prior to final.

Title: Water line setback
Wording 10' separation required between a Water line and all portions of the onsite sewage system; effluent transport line, tanks, treatment and disposal components. Location of existing water lines to be verified and relocated to provide a minimum of 10' setback to all onsite sewage system components.

Title: VIOLATION-limited permit life
Wording This permit was issued to correct a violation of WAC 246-272A. The permit must be completed within 90 days of the date of issuance.

Title: SCREEN REQUIRED
Wording A screen shall be provided either at the outlet of the septic tank or around the pump or at both locations. A pump shroud is required if only an outlet baffle filter is used.

Title: O & M Manual required
Wording Before final approval is given, the designer shall provide an operations and maintenance manual to the property owner and the Health Department. The manual must instruct the owner of the on site sewage system on the ways to properly operate and maintain all components of the system.

Wording Approval of this permit does not assure the existing septic system has capacity for all uses allowed by current code.

Title: Designer Notification/inspections
Wording Designer must be contacted prior to start of construction and for inspections during installation. DESIGNER IS REQUIRED TO DO A PRECOVER INSPECTION ON ALL TYPES OF SYSTEMS.

Title: Installation Inspection
Wording Notification of the start of construction shall be faxed or emailed to Jefferson County Public Health ONE WORKING DAY prior to start.

Title: Certified Installer Required
Wording This system must be constructed by an installer certified by the Jefferson County Public Health Department per Jefferson County Code 8.15.120 (9) and state code WAC246-272A.

Title: No Building Permit Guaranteed
Wording Approval/issuance of a sewage disposal permit or installation of a septic system does not guarantee the approval of other development or a building permit on this site. Future buildings that require connection to an on-site sewage system (OSS) shall only be approved if the OSS meets the current standards and codes in effect at the time of the building application.

Title: Code Compliance
Wording All construction and development activities must comply with all permit conditions, Washington State and Jefferson County Codes and Recommended Standards and Guidance documents in effect when the permit is issued.

Title: FOOD SERVICE OPERATIONS-NO HOLD
Wording For any alteration in the existing food service operations the Designer must submit an appropriate analysis and plan based on waste strength, water use and food service plan. The description of the proposed food service operation in the submittal indicating 'nothing will go down the drain' is inadequate and not realistic.

Title: NO RESIDENTIAL USE-NO HOLD
Wording This system is sized only for the commercial use of the building. It is not sized or approved to include any residential use of the building.

Title: Stormwater BMPs
Wording The project shall adhere to the Best Management Practices (BMPs) to control stormwater, erosion and sediment during construction. BMPs shall address permanent measures to stabilize soil exposed during construction, and in the design and operation of stormwater and drainage control systems.

Wording The application was reviewed by Jefferson County staff, via a GIS map review, for the presence of Critical Areas as described in the UDC and has concluded that the property DOES NOT contain Critical Areas. Future proposals will be subject to a new review and conditions may be added if Critical Areas are found on the property.
