Title: VIOLATION-limited permit life
Wording This permit was issued to correct a violation of WAC 246-272A for a failing drainfield. The permit must be completed within 90 days of the date of issuance. The permit expiration may be extended past 90 days if the building closes and does not generate any waste water, ie, restaurant, apartment, or other sources.

Title: O & M Manual required
Wording Before final approval is given, the designer shall provide an operations and maintenance manual to the property owner and the Health Department. The manual must instruct the owner of the on site sewage system on the ways to properly operate and maintain all components of the system.

Wording This system must be constructed by an installer certified by the Jefferson County Public Health Department per Jefferson County Code 8.15.120 (9) and state code WAC246-272A-0250 due to distance to shoreline.

Title: No Building Permit Guaranteed
Wording Approval/issuance of a sewage disposal permit or installation of a septic system does not guarantee the approval of other development or a building permit on this site. Future buildings that require connection to an on-site sewage system (OSS) shall only be approved if the OSS meets the current standards and codes in effect at the time of the building application.

Title: Code Compliance
Wording All construction and development activities must comply with all permit conditions, Washington State and Jefferson County Codes and Recommended Standards and Guidance documents in effect when the permit is issued.

Title: Contaminated Material - onsite
Wording H - The material that is removed from the system that has come into contact with waste water must be treated/decontaminated prior to final disposal. The material from the system shall be removed and placed as described in the design to allow for pathogen die-off. The material shall be placed in lifts of no more than 6"-8", each lift covered with lime. It shall be covered and protected from erosion, covered with 6 inches of uncontaminated soil or other approved material.)

Title: CRICTICAL AREAS CONDITION
Wording The application was reviewed by the Jefferson County staff for the potential presence of Critical Areas under the provisions of the Unified Development Code (UDC). After a Geographic Information Systems mapping review, the following Critical Areas were confirmed to be present on the subject property: geologically hazardous areas (unstable shoreline slope stability, seismic); fish and wildlife habitat conservation area (Type N stream; functionally isolated from marine water); wetlands (functionally isolated); critical aquifer recharge area (susceptible and special aquifer recharge protection area); and seawater intrusion protection zone (coastal). The site plan shows the drainfield well beyond the 50-foot buffer stream buffer so no habitat managment plan was required. A Geology Hazard Report, prepared by Stratum Group (dated September 26, 2017), concluded that the proposal would not affect slope stability. An Aquifer Recharge Area Report, prepared by Streamline Environmental (dated September 2017), stated that groundwater generally flow east and southeast towards Port Townsend Bay.
