THE PROPERTY OWNER SHALL ASSURE THAT MONITORING IS PROVIDED BY AN APPROVED ENTITY AT THE FREQUENCY DEFINED PER STATE WAC 246-272A AND JEFFERSON COUNTY CODE 8.15 AS ADOPTED OR AMENDED. A COPY OF THE RECORDED OPERATIONS AND MONITORING AGREEMENT IS REQUIRED PRIOR TO FINAL APPROVAL OF THE SEWAGE DISPOSAL PERMIT

Title: domestic strength waste
Wording This onsite sewage system is designed for domestic strength wastewater only. Disposal of any other waste strength is considered a violation of this permit.

Title: BOD-WASTE STRENGTH MONITORING
Wording Due to the fact that the system is proposed to serve a commercial kitchen permitted for food processing waste strength (BOD, TSS, Grease and oils) is to be monitored annually in addition to the standard annual monitoring inspection of the system. This information will verify the system is working as intended and will provide the ability for possible expansion in the future. Responsibility is that of the owner. Results are to be submitted to the Jefferson County Health Dept.

Title: design flow/water usage monitorin
Wording Exceeding the permitted design flow of 720 gal/day shall constitute a violation of this permit. Water usage monitoring should be evaluated regularly to ensure compliance and avoid hydraulic overload of the system.

Title: risers
Wording Risers are required to grade with secured lids over both compartments and over the outlet of the septic tank for inspection and maintenance/monitoring. Minimum 20" riser over inlet.

Title: protective barriers
Wording H - Permanent barriers are required along/around primary and reserve drainfield areas to protect from parking, driving, and other land disturbing activities prior to final.

Title: Water line setback
Wording 10' separation required between a Water line and all portions of the onsite sewage system; effluent transport line, tanks, treatment and disposal components.

Title: O & M Manual required
Wording Before final approval is given, the designer shall provide an operations and maintenance manual to the property owner and the Health Department. The manual must instruct the owner of the on site sewage system on the ways to properly operate and maintain all components of the system.

Title: System capacity/uses
Wording Approval of this permit does not assure the existing septic system has capacity for all uses allowed by current code.

Title: Designer Notification/inspections
Wording Designer must be contacted prior to start of construction and for inspections during installation. DESIGNER IS REQUIRED TO DO A PRECOVER INSPECTION ON ALL TYPES OF SYSTEMS.

Title: Installation Inspection
Wording Notification of the start of construction shall be faxed or emailed to Jefferson County Public Health ONE WORKING DAY prior to start.

Title: -HOLD-INSPECT STRUCTURES
Wording H - Prior to final of this septic permit the waterlines to structures where plumbing had been installed and now removed must be inspected to verify no water enters the structures lines are removed to beyond the building structures. Contact JCPH at 360-385-9444 to schedule inspection or can be done at time of final pressure test.

Title: CONFIRMED CRITICAL AREAS
Wording The application was reviewed by the Jefferson County Department of Community Development staff on August 2nd, 2017 for the potential presence of Critical Areas under the provisions of the Unified Development Code (UDC). After an initial Geographic Information Systems mapping review and an investigative site inspection, the following Critical Areas were confirmed to be present on the subject property: Wetlands, seismic hazard area, FEMA flood zone, Special Aquifer Recharge Protection Area (SARPA) and Susceptible Aquifer Recharge Area.

Title: APPROVAL FOR NEW SEPTIC ONLY
Wording This approval is for a new commercial septic system only. Any future permits on this site are subject to review for consistency with applicable codes and ordinances and does not preclude review and conditions which may be placed upon future permits.

Title: SITE PLAN CHANGES
Wording The site plan submitted with the new septic system application on June 26, 2017 and revised sheets submitted 8/6/18 have been reviewed for consistency under the UDC, and has been approved by Jefferson County Department of Community Development. Any modifications, changes, and/or additions to the approved site plan shall be resubmitted for review and approval by Jefferson County Department of Community Development.

Title: ADMINISTRATIVE BUFFER REDUCTION
Wording It has been determined by the UDC Administrator that the wetland buffer of 110-feet as described in the Wetland Delineation Report completed by Westech Company June 2016 is reduced to 75% of the buffer width. The reduced buffer shall be at least 82.5-feet. The reduced buffer and minor encroachments into the buffer from the proposed septic system shall be considered offset by the existing ecosystem enhancements established on the property.

Title: PURPOSE FOR DESIGN
Wording This septic system is designed to serve a commercial kitchen with design flows estimated at 360 gallons per day and a 3 bedroom home for a total septic capacity of 720 gallons per day. The commercial kitchen is to be permitted by WSDA for food processing of pesto, fruit jam, apple cider syrup, dried herbs and salt. The system was sized larger than required by the current activities to allow for future expansion from 2 employees 2 days per week as described.

Title: ECOLOGY APPROVED HOLDING TANK
Wording There is an existing structure permitted under BLD18-00035 which houses a poultry processing facility permitted by WSDA and a farm stand. The wastewater from this building is disposed of in a facility approved by Ecology which consists of a holding tank which is pumped out and hauled to a composting facility on adjacent property.
