Title: ONGOING MONITORING
Wording H - AS PER WAC 246-272A AND JEFFERSON COUNTY CODE 8.15 ALL SYSTEMS REQUIRE MONITORING. THIS SEWAGE DISPOSAL SYSTEM WILL REQUIRE AN ACTIVE MONITORING AGREEMENT RECORDED TO THE PROPERTY TITLE PRIOR TO FINAL APPROVAL OF THE SEWAGE DISPOSAL SYSTEM.

Title: pressure test observation
Wording Health Dept. required to observe pump test with system designer when system fully installed/complete, 48 hours notice to be provided for scheduling.

Title: reserve area, alternative
Wording When/if designated reserve area is utilized an alternative system providing an additional level of treatment may be required.

Title: Tank Abandonment
Wording H - Existing tank shall be properly abandoned. It shall be pumped and filled with clean fill. Documentation to be provided to Health Dept. prior to final.

Wording All components of the septic system are to be completely protected from vehicular traffic or mechanical disturbance.

Title: community drainfield
Wording This drainfield, serving more than one residence, will require a management and operations agreement with an entity approved by the Jefferson County Health Department as per Jefferson County Policy when/if the residences are separately owned or the property is subdivided.

Wording Approval/issuance of a sewage disposal permit does not guarantee the approval of other development or a building permit on this site. Compliance with other Jefferson County and Washington State Codes is required.

Wording This onsite sewage system is designed for domestic strength wastewater only. Disposal of any other waste strength is considered a violation of this permit.

Wording Risers are required to grade with secured lids over both compartments and over the outlet of the septic tank for inspection and maintenance/monitoring. Minimum 20" riser over inlet.

Title: MONITORING SCHEDULE
Wording As per Jefferson County Code 8.15 all onsite sewage systems require monitoring. Monitoring shall be provided by an approved entity. THIS SYSTEM IS REQUIRED TO BE INSPECTED PER JEFFERSON COUNTY CODE 8.15 AS ADOPTED OR AMENDED.

Wording H - An asbuilt drawing and certification of completion by the Designer is required prior to final approval.

Title: riser on d-box
Wording Distribution boxes shall be fitted with a riser to grade, constructed of durable materials and a secured lid, adequately sized to allow visible inspection of the liquid level in the box.

Wording 10' separation required between a Water line and all portions of the onsite sewage system; effluent transport line, tanks, treatment and disposal components.

Title: VIOLATION-limited permit life
Wording This permit was issued to correct a violation of WAC 246-272A. The permit must be completed within 90 days of the date of issuance.

Title: PUMP CHAMBER
Wording Pump chamber shall be water tight and a minimum capacity of 1000 g. It shall be equipped with a control panel outside within line of sight of the pump chamber. The panel shall have an audio and visual alarm, a cycle counter, elapsed time meter and a riser to grade. It shall be UL listed and meet L&I requirements.

Title: SCREEN REQUIRED
Wording A screen shall be provided either at the outlet of the septic tank or around the pump or at both locations. A pump shroud is required if only an outlet baffle filter is used.

Title: System capacity/uses
Wording This system designed at 360 GPD is proposed as a repair to serve three one bedroom cabins. This does not meet the minimum requirement of 240 gal/day per residentical unit therefore no expansions of square footage or footprint are permitable. Approval of this permit does not assure the existing septic system has capacity for all uses allowed by current code.

Title: Designer Notification/inspections
Wording Designer must be contacted prior to start of construction and for inspections during installation. DESIGNER IS REQUIRED TO DO A PRECOVER INSPECTION ON ALL TYPES OF SYSTEMS.

Title: Installation Inspection
Wording Notification of the start of construction shall be faxed or emailed to Jefferson County Public Health ONE WORKING DAY prior to start.

Title: SYSTEM CONNECTION-NO HOLD
Wording This permit is for addition of a pump chamber to use an existing onsite system not currently in use that was permitted under SEP91-160. The septic tank currently being used, that was permitted under SEP92-90, will continue to remain in service if it is found to be in good condition.

Title: CONNECTIONS TO SYSTEM-HOLD
Wording H - Prior to final approval of this permit it shall be verified if the Cabins at #33 and #51 are connected to this system. Current records only show #41 connected to the tank that is being connected to the drainfield that is being used which was permitted under SEP91-160.
