Wording AS PER WAC 246-272 ALL ALTERNATIVE SYSTEMS REQUIRE MONITORING. THIS MONITORING IS PROVIDED BY AGREEMENT BETWEEN THE JEFFERSON COUNTY ENVIRONMENTAL HEALTH DIVISION AND THE JEFFERSON COUNTY PUBLIC UTILITY DISTRICT #1. THIS SEWAGE DISPOSAL SYSTEM WILL REQUIRE AN ACTIVE MONITORING CONTRACT WITH THE PUD #1 PRIOR TO FINAL APPROVAL OF THE SEWAGE DISPOSAL SYSTEM.

Wording Health Dept. required to observe pressure test when system fully installed/complete, 48 hours notice to be provided for scheduling.

Wording An asbuilt drawing and certification of completion by the designer is required prior to final approval.

Title: domestic strength waste
Wording This onsite sewage system is designed for domestic strength wastewater only. Disposal of any other waste strength is considered a violation of this permit.

Title: BOD-WASTE STRENGTH MONITORING
Wording Waste Strength (BOD, TSS, Grease and oils) is to be monitored if a commercial use producing high strength waste is discharged to the system. Responsibility is that of the owner. Results are to be submitted to the Jefferson County Health Dept.

Title: design flow/water usage monitorin
Wording Exceeding the permitted design flow of 732 gal/day shall constitute a violation of this permit. Water usage monitoring shouldl be conducted daily for the first two months & weekly thereafter. These monitoring results shall be available to the Jefferson County Health Dept. upon request.

Title: risers
Wording Risers are required to grade with secured lids over the inlet and outlet of the septic tank. Minimum 24" riser over inlet.

Title: AEROBIC TREATMENT SYS MANAGEMENT
Wording This onsite sewage system is permitted using an aerobic treatment unit (ATU) with UV disinfection. All ATU systems must have a continuous and perpetual management agreement in place for the life of the system. This management shall be provided by the Jefferson County PUD #1. A signed contract with PUD shall be required prior to final approval.

Title: ATU maintenance
Wording Maintenance of the system shall be completed by a manufacturers authorized representative at the frequency required by the manufacturer. That frequency may be increased to meet State requirements for Treatment Standard 1 systems.

Title: ATU contract recordng/ compliance
Wording The signed maintenance contract with the manufacturer's authorized representative shall be recorded to the property title prior to final approval. Non-compliance with the contract or maintenance requirements is a violation of this permit.

Title: surface water setback
Wording Setback to surface waters- 100' required from drainfield and reserve areas. 50' required from tanks.

Title: protective barriers
Wording Permanent barriers are required along/around septic and pump tank to protect from parking, driving, and other land disturbing activities prior to final, unless tanks are installed with traffic rating

Title: WATER LINE CROSSING-NO HOLD
Wording All portions of the drainfield must maintain 10' min. setback from water lines. Where water line and transport line cross, follow requirements per Department of Ecology Sewage Works Design.

Title: -REDUCED SETBACK-NO HOLD
Wording Tanks are permitted with a 4'' min., reduced setback from the foundation and a 2' setback from Water St. per the plan.

Title: ENCOUNTERING EXISTING SYS-NO HOLD
Wording If any portion of an existing drainfield is uncovered during excavation of this system, STOP work, and contact the engineer and the Health Department.

Title: ACHEOLOGICAL ARTIFACTS-NO HOLD
Wording The area is known to have historical/cultural deposits. In the event of discovery of such deposits, including shell middens, work shall CEASE. Contact the engineer and the Health Department.

Title: SHORELINE MASTER PROGRAM
Wording This proposal has been reviewed by Development Review Division staff and meets Unified Development Code and Shoreline Master Program review requirements as they relate to the expansion of on-site septic systems. Further review related to this proposal will occur at the time of building permit or change of use applicaiton.

Title: Stormwater BMPs
Wording The project shall adhere to the Best Management Practices (BMPs) to control stormwater, erosion and sediment during construction. BMPs shall address permanent measures to stabilize soil exposed during construction, and in the design and operation of stormwater and drainage control systems.
