Title: PERMIT ON SITE
Wording This permit must be located on site when any work encompassed by the permit is being performed. Failure to do so may result in suspension of all work.

Title: RESPONSIBLE
Wording The applicant shall be responsible for the cost to repair any and all damage to the county roadway and rights-of-way, including the integrity of all survey monuments or boundary markers.

Title: WORKER/ROW
Wording Any time that workers are on the county right-of-way warning signs shall be posted that workers are present. All workers shall comply with applicable Labor and Industries Guidelines for clothing including reflective vests. Any time that equipment must enter onto the right-of-way, signing and flaggers shall be used in accordance with applicable MUTCD standards.

Wording The boardwalk must comply with and meet all applicable conditions and standards required under the current American Disability Act (ADA).

Title: WHEEL STOPS
Wording Public Works recommends the installation of wheel stops along the segment of N. Water St. adjacent to the Ajax Cafe's boardwalk. Please be aware that parking within County right-of-way is not dedicated exclusively for the use of Ajax Cafe customers or employees and can be used by members of the public, in general, at any given time. Storage of vehicles or other such items is not allowed.

Title: MAINTENANCE AGREEMENT
Wording The Applicant must agree to and have signed and recorded the attached Maintenance Agreement and provide a copy to Jefferson County Public Works.

Title: BOARDWALK REBUILD
Wording The proposed boardwalk replaces the existing structure which shall not be rebuilt or expanded outside the existing footprint.

Wording The boardwalk must comply with and meet all applicable conditions and standards required under the current International Building Code (IBC).

Title: MAINTENANCE
Wording The Owners of the Ajax Cafe must inspect and maintain the boardwalk/deck on a continual basis so it is keep in a functional and safe condition for use by the public.

Title: PUBLIC ROW
Wording The boardwalk is located within Jefferson County right-of-way, and as such, cannot be closed off or blocked prohibiting public use.

Title: PRE-CON
Wording Applicant must give Jefferson County Public Works office (360-385-9159) a minimum of 7 working days notice prior to beginning work. An on-site pre-construction conference shall be scheduled by the applicant and will include Public Works and the contractor who will be performing the work. Prior to this meeting, any utility locate marks shall be done (Call 811-48 hours before you dig).
