This file was incorrectly attached to another parcel so we created another case SEP77-300 based on the installer John Shold researched his files and found what appears to be an original record with signatures of applicant and owner (no HD signatures) which he provided to this office. The Health Department did sign off on a building permit for a 2 bedroom residence in 1976. 4/16/10 LAM did an inspeciton of uncovered portions of the system to confirm that the system was installed consistent with the original proposal and finaled the permit. SEP77-300 is now cancelled and all info filed to SEP76-126. lam
